• The risk of endometrial hyperplasia progressing to carcinoma is related to the presence and severity of cytologic atypia.
• Most of the hyperplasias seemed to remain stable (18%) or regress (74%).
• The premalignant potential is influenced by age, underlying ovarian diseases, endocrinopathy, obesity, and exogenous hormone exposure.

¥ Sonographic Findings R/O Endometrial Hyperplasia:

à ET > 10 mm in menopaused women

* for normal menopaused female:

if HRT (+), ET 6-10 mm

if HRT (-), ET 4-8 mm

¥ Treatment: Progestin Therapy

* 用法:

• In cases with no atypia and no ovulation induction:

? cyclical progestin therapy:

eg. provera (5) 10-20 mg/day per month

‚ continuous progestin therapy:

eg. megace (20) 20-40 mg qd

• In cases with atypia:

à contious pregestin therapy is favored:

eg. megace (20) 40 mg qd

* F/U:

Repeat endometrial bx. 2-3 months following medical treatment.
